Section 1: Guest Information

The questions below should be answered by the individuals who are taking part in a program/activity at WinSport facilities. Legal Guardians may assist individuals who are not able to fill out the questionnaire on their own.

If you answer “yes” to any of the questions below the attendee must not attend the program/activity or WinSport’s facility. Use the AHS Online Assessment Tool to determine whether isolation is required or testing is recommended.

Section 2: Symptom Reporting

2.1 Does the attendee currently have any of the following symptoms:

  • Fever (temp above 38C or above)
  • Cough
  • Shortness of breath/Difficulty breathing
  • Loss of sense of taste or smell
  • Sore throat/painful swallowing
  • Chills
  • Runny nose/Nasal congestion
  • Feeling unwell/Fatigue
  • Nausea/Vomiting/Diarrhea
  • Unexplained loss of appetite
  • Muscle/Joint aches
  • Headache
  • Conjunctivitis (pink eye)
2.1 Answer
Section 3: Exposure Reporting

If the attendee answers "yes" to any of the questions below, they cannot enter the facility or participate in activities. Go home and use the AHS Online Assessment Tool to determine if testing is recommended.

3.1 Has the attendee, or someone who is currently living or staying at their home, travelled outside of Canada in the last 14 days?
3.2 Has the attendee had “close contact”* (face-to-face contact within 2 metres or direct physical contact such as hugging) in the last 14 days with a person who has a confirmed case of COVID-19?
3.3 Has the attendee had “close contact”* (face-to-face contact within 2 metres or direct physical contact such as hugging) with a person who is currently scheduled for a COVID-19 test or who is waiting for the results of a COVID-19 test?
3.4 Is the attendee currently waiting for a scheduled COVID-19 test or the results of a COVID-19 test?
3.5 Has the attendee been directed by AHS or their school to self-isolate or quarantine for a period of time that has not yet expired?
3.6 In the past 14 days, has the attendee or a member of their household been informed that they are a close contact of someone who tested positive for a COVID-19 variant?

*A health care worker in an occupational setting wearing the recommended PPE is not considered a “close contact”
